
FINANCIAL:
We accept payments by cash, check, or credit cards
(Master card or Visa). Co-pays and deductibles are due
at the time of visit.
Prior balances must be paid at the time of visit. If you
are unable to pay the full balance, we ask that you pay
at least $50 or 20% of the balance (whichever is higher)
prior to being seen.
For cash pay patients, full payment is due at the time
of the visit.
MEDICATIONS:
Please bring all current medications or a list of
current medications at the time of your visit.
Please bring a list of prescriptions that need to be
renewed at the time of your visit. This will prevent any
delays in getting your medications.
For mail order pharmacies, we will give the patient the
prescription to mail away. We do not fax prescriptions
in to mail order pharmacies for liability reasons.
For call in / fax in prescription refill requests,
please allow 72 hours to process the request. Please
plan ahead, so that you do not run out of medications.
APPOINTMENTS:
Patients are seen by appointment only, HOWEVER, If you
are sick and need to be seen urgently, we will see you
on the same day.
Please arrive on time for your visit. For new patients,
please arrive 30 minutes prior to your visit to fill out
initial paperwork.
If you are more than 30 minutes late for an appointment,
the appointment must be rescheduled.
Please give a notice of at least 24 hours if you have to
cancel or reschedule your appointment.
No-shows will be charged $10 for every time they miss a
regular appointment without prior cancellation, unless
it was a bona fide emergency. Patients who are no shows
for procedures will be charged $50. Please note that you
will be responsible for these charges as insurance will
not pay for this charge.
For patients who are no-shows three or more times, it is
our policy to discharge them from our practice.
REFERRALS:
As a courtesy, we will refer you to other health care
providers, as needed. However, please understand that
when we advise you to see another provider or to have a
test, it is ultimately your responsibility to see that
it gets done.
